Step 1	Fill in the form above starting from "Booking accomodation in Zakopane". Skip the credit card form.
Step 2	Submit the form electronically by pressing this (reset to clear the form and start again)
Step 3	Print out the form and check the printout again. If your electronic submission failed (you received a failure report) or bounced back (you received an e-mail from the mail delivery system) then fax *PAGE ONE* and *PAGE TWO* of the form to +48 22 822 2387, ATTN: Konrad Bajer. This is important. Note that the position of page breaks may vary. Make sure that *END OF PAGE TWO* is included. Keep a copy of the entire form for your records.
Step 4	On the printed out form NOT on the computer fill in all your credit card details in your own hand and sign. If you are not paying at the time of booking fill in "Name", "Date" and sign. If you are paying by bank transfer fill in the "Amount" and write "BANK TRANSFER" instead of the credit card number (see more info). Please note that all payments must be made in Polish Zlotys. The exact prices on the form above are in Zlotys. The equivalent amounts in $ are approximate. If you want to calculate more precisely the charge on your credit card check the daily exchange rates here

Step 6	Keep the page you have just faxed to TRIP and bring it to Zakopane
Step 7	Enter below the total sum you have just paid to TRIP and press Submit. If you are not paying at the time of registration, please enter text "SENT FAX TO TRIP BUT" followed by explanation, e.g., "I will pay later", "I don't have a credit card", "I would like to pay by bank transfer", etc. If you want to pay by bank transfer the details are here. Step 8	You can always update your registration data. If anything changes, please submit the registration form again filling in just the updated fields. For example, if you haven't booked your flight by the time your register you should do it later by filling in the field *Your travel arrangements* only and resubmitting the form (Step 2). Remember to complete Step 7 immediately after making the payment.

NOTE

Please use this form to book accomodation and special events for you and the accompanying person(s). Book by entering in the appropriate boxes the number of rooms/tickets you want. Leave other boxes empty. First read the instructions above (Step 1 - Step 7).
There is no conference fee to pay.
In order to book the hotel rooms you have to pay a deposit equal to the charge for one night in the accomodation category you have chosen.
The deposit must be paid before 1 July.
You have to pay before 1 August for all special events you booked (except the drink party which is free but should be booked).
If you obtain financial support from the organisers the deposit you pay when booking accomodation will either be refunded or, if you wish, credited towards other expenses, e.g., special events. It will not be refunded if you don't come. No financial support can be transferred to you in advance.
If you are certain that you are coming to Zakopane, but you are unable to pay the deposit, e.g. you have no credit card, please e-mail me explaining your circumstances.
